About the role

Provide comprehensive patient care in the home health setting. Key responsibilities include conducting in-person patient assessments, managing and overseeing care plans with caregivers, communicating with healthcare providers, and educating patients and families on home health procedures such as wound care and medication management. The role aims to reduce readmissions through preventative care and care continuity.

Requirements

• Current RN license in the applicable state
• CPR/BLS/First Aid certification
• Reliable transportation
• At least one year of home health RN experience

• OASIS certification or experience
• Practical trach/ventilator experience
