RN-Case Manager

We are seeking a skilled nurse specializing in discharge planning and case management. Responsibilities Include Assessing patient needs and developing discharge plans in collaboration with interdisciplinary teams.Ensuring compliance with discharge planning regulations and identifying barriers to timely discharge.Managing high-risk patient cases, including root cause analysis for readmissions, and coordinating with post-acute providers.Participating in quality improvement initiatives and potentially overseeing utilization management, ED case management, and admission processes.Applying evidence-based practices and adhering to professional nursing standards, including the ANA Code of Ethics. Qualifications And Skills Licensed Registered Nurse in applicable states with valid certifications.Minimum of three years of acute care experience.Strong assessment, communication, and coordination skills.Physical ability to perform tasks such as lifting, standing, walking, and data entry.
