Responsibilities
- Conduct initial client assessments and continuously refine care plans
- Provide hands-on nursing care and patient education
- Maintain open lines of communication with patients, families, and the healthcare team
- Manage patient care coordination for your caseload
- Participate in interdisciplinary team meetings to ensure comprehensive care planning
Qualifications
- Licensed as a Registered Nurse / RN in the state you work in
- Proficiency with Electronic Medical Records (EMR) software
- Excellent planning, organizing, and communication skills
- Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards
- Valid Driver's License and reliable vehicle with current insurance
