RN Case Manager

About the role

The RN Case Manager is responsible for providing comprehensive care coordination and support to patients in a healthcare setting. This includes managing patient care plans, ensuring compliance with healthcare regulations, and collaborating with interdisciplinary teams to improve patient outcomes.

Responsibilities

• Develop and implement individualized care plans for patients
• Monitor patient progress and adjust care plans as necessary
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure seamless care delivery
• Ensure all care is provided in accordance with regulatory standards and best practices
• Communicate effectively with patients and their families to provide emotional and informational support
• Participate in quality improvement initiatives to enhance patient care

Requirements

• Bachelor's degree in Nursing or related field
• Current RN license in good standing
• At least 2 years of relevant clinical experience
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Qualifications

• Experience in case management or a related field
• Knowledge of healthcare policies and procedures
• Proficiency in Microsoft Office Suite

Pay

$45,000 - $55,000 annually
