RN Case Manager

About the role

The RN Case Manager is responsible for providing comprehensive care coordination and clinical oversight to patients in a healthcare setting. This includes managing patient care plans, ensuring compliance with healthcare policies and regulations, and collaborating with interdisciplinary teams to improve patient outcomes.

Responsibilities

• Develop and implement individualized care plans for assigned patients
• Monitor patient progress and adjust care plans as necessary
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure seamless patient care
• Ensure all care activities are performed in accordance with regulatory standards and best practices
• Document patient interactions and care activities accurately and timely
• Participate in quality improvement initiatives to enhance patient care

Requirements

• Bachelor's degree in Nursing or related field
• Current RN license in Missouri
• Minimum 2 years of relevant clinical experience
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Pay

$50,000 - $60,000 annually
