About the role
Coordinate care for diverse, underserved patients across hospital and community settings.
Responsibilities
- Conduct comprehensive patient assessments to identify medical, behavioral health, and social needs.
- Develop and implement individualized, patient-centered care plans.
- Facilitate smooth transitions of care between settings.
- Collaborate with an interdisciplinary team to ensure holistic patient support.
- Promote health equity and participate in quality improvement initiatives.
- Utilize community resources effectively to enhance patient outcomes.
- Mentor staff and learners within a supportive, team-oriented environment.
Requirements
- Registered Nurse (RN) licensure required.
- Experience in care coordination, case management, or related roles preferred.
- Strong assessment, communication, and collaboration skills.
- Commitment to health equity and patient advocacy.
- Ability to work effectively with diverse populations and interdisciplinary teams.
