About the role
Coordinate care for diverse, underserved patients across hospital and community settings.
Responsibilities

Requirements
- Registered Nurse (RN) licensure required.
- Experience in care coordination, case management, or related roles preferred.
- Strong assessment, communication, and collaboration skills.
- Commitment to health equity and patient advocacy.
- Ability to work effectively with diverse populations and interdisciplinary teams.
