RN Case Manager

About the role

The RN Case Manager at Medical City Dallas plays a pivotal role in patient care, leveraging clinical expertise to ensure high-quality, patient-centered care. They are responsible for coordinating the plan of care, managing the length of stay, and facilitating a smooth transition to post-acute care.

Responsibilities

• Comprehensively assesses psychosocial, medical, and discharge needs of patients and families
• Reassesses patient conditions as needed, considering readmission risks and developing mitigation strategies
• Collaborates with multidisciplinary teams to develop and implement discharge plans
• Ensures appropriate post-acute care and resources are in place
• Evaluates patient progress and makes referrals to third-party payer programs and case management services
• Facilitates efficient patient throughput and care transitions
• Documents care coordination activities and communicates with healthcare team members
• Liaises with various stakeholders to address patient needs and concerns
• Directs activities to meet the needs of under-resourced patient populations

Requirements

• Registered Nurse with current Texas state license
• Associate Degree in Nursing or Nursing diploma required, Bachelor's Degree in Nursing preferred
• 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required
• Certification in case management preferred
• InterQual experience preferred

Qualifications

• Knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives
• Ability to advocate for patient rights, needs, and values

Skills

• Strong clinical judgment and decision-making skills
• Excellent communication and interpersonal skills
• Effective problem-solving and conflict resolution abilities
• Proficiency in InterQual and related quality improvement tools
