About the role
The RN Case Manager at Medical City Dallas plays a pivotal role in patient care, leveraging clinical expertise to ensure high-quality, patient-centered care. They are responsible for coordinating the plan of care, managing the length of stay, and facilitating a smooth transition to post-acute care.
Responsibilities
- Comprehensively assesses psychosocial, medical, and discharge needs of patients and families
- Reassesses patient conditions as needed, considering readmission risks and developing mitigation strategies
- Collaborates with multidisciplinary teams to develop and implement discharge plans
- Ensures appropriate post-acute care and resources are in place
- Evaluates patient progress and makes referrals to third-party payer programs and case management services
- Facilitates efficient patient throughput and care transitions
- Documents care coordination activities and communicates with healthcare team members
- Liaises with various stakeholders to address patient needs and concerns
- Directs activities to meet the needs of under-resourced patient populations
Requirements
- Registered Nurse with current Texas state license
- Associate Degree in Nursing or Nursing diploma required, Bachelor's Degree in Nursing preferred
- 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required
- Certification in case management preferred
- InterQual experience preferred
Qualifications
- Knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives
- Ability to advocate for patient rights, needs, and values
Skills
- Strong clinical judgment and decision-making skills
- Excellent communication and interpersonal skills
- Effective problem-solving and conflict resolution abilities
- Proficiency in InterQual and related quality improvement tools
