About the Role
This position involves assessing, developing, and implementing patient discharge plans in collaboration with an interdisciplinary team to ensure safe transition to the next level of care. The nurse ensures compliance with discharge planning standards, identifies barriers affecting length of stay, and evaluates high-risk patients for readmission, developing targeted care plans. Responsibilities may include utilization management, emergency department case management, admissions, and acting as a liaison with post-acute providers. The role requires applying evidence-based practices, adhering to professional nursing standards, and maintaining patient safety.
Responsibilities
- Evaluate discharge needs and barriers.
- Coordinate care with multidisciplinary teams and post-acute providers.
- Ensure compliance with healthcare regulations and standards.
- Support quality improvement initiatives.
- Perform other assigned duties to support patient care and safety.
Requirements
- Graduate of an accredited nursing program or equivalent.
- Minimum three years of acute care experience.
- Active RN license in relevant states or compact license.
- Strong assessment, communication, and collaboration skills.
- Physical ability to perform job duties, including lifting and mobility tasks.
