RN Case Manager

Make a lasting difference as a compassionate Registered Nurse (RN) supporting patients and families through meaningful moments of care. Legacy Hospice is seeking a Full-Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care in Fishersville.

In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Partner closely with our Clinical Director to assess the eligibility and suitability of clients for specialized hospice services.
• Provide direct care as an RN Case Manager, ensuring patient comfort and quality of life remain paramount.
• Deliver top-notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
• Empower patients and families through education, encouraging their active participation in creating personalized care plans.
• Participate in call duties to ensure support is available when it matters most.

Requirements

• Current unencumbered license as a Registered Nurse.
• Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting (hospice experience a plus).
• Computer proficiency, including experience with EMR systems.
• Current CPR certification (if required by state).
• Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay

$80,000 - $90,000 per year (average pay range). Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position.

Schedule

Monday - Friday, 8:00 AM - 4:30 PM.
