Hospice Registered Nurse (RN) Case Manager – Full-Time | Vernon Hills, IL JourneyCare Hospice Tired of high caseloads, frequent weekend rotations, and feeling stretched too thin? We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Vernon Hills area. Schedule & Territory Territory: Vernon Hills, 60061 and surrounding communities.Monday–Friday, 8:30 AM – 5:00 PM. No On-Call.One weekend rotation every 12 weeks.
