RN Case Manager

About the Role

The RN Case Manager serves as a direct link between medical services and ICF/IRA residents, staff, and external clinical professionals. This role is responsible for complete nursing care and routine medical procedures within the scope of nursing practice, including supervising LPNs, AMAP, and Direct Support Professionals. The RN must possess a working knowledge of OPWDD rules and regulations, JBFCS policies, and the ability to work effectively with diverse populations.

Responsibilities

• Provide direct nursing care as prescribed by a physician or Nurse Practitioner, adhering to recognized nursing standards and State/Federal regulations.
• Use the nursing process to assess the total health needs of individuals with developmental disabilities.
• Maintain direct interaction with consumers at least once per week.
• Perform nursing assessments through direct physical examination upon program entry and quarterly thereafter.
• Develop and implement individualized nursing service plans, updating them annually or when a consumer’s condition changes significantly.
• Coordinate acute and ongoing health services as required by need and regulatory mandate.
• Monitor health and behavioral status, labs, and treatment efficacy, ensuring timely communication of findings to clinical personnel.
• Ensure all medical record documentation is accurate, timely, and outcome-based.
• Provide initial and ongoing training to staff in nursing tasks, health/hygiene methodologies, communicable disease control, illness detection, first aid, and client-specific health issues (e.g., seizure management).
• Train new hires and per diem staff in the above areas before they assume client-specific work assignments.
• Provide training and support to direct care staff on medication administration, tube feeding, and diabetic care using OPWDD-approved curricula.
• Conduct annual clinical performance evaluations for unlicensed direct support staff on procedures such as medication administration, tube feeding, and insulin administration.
• Maintain documentation of all training curricula, attendance, and post-training testing.
• Work with electronic health record management, care coordination, and interface with external service providers.
• Actively participate in the Interdisciplinary Team (IDT).
• Perform additional duties as assigned.

Requirements

• Active, unrestricted New York State license as a Registered Professional Nurse.
• Bachelor’s degree and a minimum of 3 years of clinical experience in a hospital or primary care setting (preferred).
• Experience with the intellectually disabled population (preferred).
• Managerial experience (preferred).
• Excellent verbal and written communication skills.
• Ability to work in a team environment and independently, with sound judgment and decision-making skills.
• Proficiency in Microsoft Office Suite applications.

Work Environment

The work environment involves no physical risk or hazardous conditions. The role requires sitting (50% of the time) and standing (50% of the time).
