RN Case Manager

Responsibilities

• Assess, plan, implement, and evaluate patient care in collaboration with healthcare teams.
• Cookordinate and manage patient case load to ensure timely and appropriate care delivery.
• Communicate effectively with patients, families, and healthcare providers to facilitate care planning and delivery.
• Maintain accurate and detailed patient records in compliance with company and regulatory standards.
• Provide patient education and support to enhance health outcomes.
• Advocate for patients’ needs and coordinate community resources as needed.

Requirements

• A current and valid RN license in the state of Maine.
• Previous case management or nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage multiple priorities.
• Reliable transportation to visit patients in Waterboro, ME area.

Pay
