RN Case Manager

About the role

As an RN Visit Nurse, you'll support a full range of patient services to bring comfort and dignity to our clients in their homes. This role offers truly flexible scheduling, 1:1 patient care, and the opportunity to change lives every day through compassionate, patient-centered nursing.

Responsibilities

• Provide in-home direct patient care according to the provider's prescribed plan of care and within nursing scope of practice, while closely monitoring progress and recovery.
• Assess and document patient conditions (vitals, blood/stool/urine, glucose) and report changes as needed.
• Administer medication, insulin, and IV/fluids, documenting thoroughly.
• Inspect and care for wounds, change dressings, and assist with personal hygiene.
• Assess patients for injuries, infections, bedsores, and other conditions, delivering appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

Requirements

• Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required.
• Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to and from care sites and work locations.

Qualifications

• Practical trach and/or ventilator experience preferred but not required.
