Competitive payCompany vehicle provided for work-related visitsBenefits available to eligible employeesSupportive team environmentOpportunity to make a meaningful impact in patients’ livesWork in a community-based setting while helping patients remain safely at home Position Responsibilities Complete patient assessments and develop individualized plans of care.Coordinate and oversee patient care to ensure quality and continuity of services.Monitor patient progress and communicate changes in condition to the appropriate healthcare professionals.Conduct scheduled home visits throughout Bloomington and surrounding areas.Collaborate with physicians, patients, families, and the care team.Maintain accurate, timely clinical documentation in accordance with agency policies and applicable regulations.Educate patients and caregivers regarding care plans, medications, and health-related needs.Help ensure services are delivered in accordance with established clinical standards and home health requirements. Qualifications Current, unrestricted Registered Nurse (RN) license in Indiana.Strong communication, organization, and time-management skills.Compassionate, dependable, and patient-focused approach to care.Ability to work independently while collaborating with a multidisciplinary team.Valid driver’s license and ability to meet company vehicle insurance requirements.Home health experience and case management experience preferred.
