RN Case Manager

Provides case management and nursing services to Hudson Valley Hospice patients and their families, ensuring services align with the agency’s philosophy, goals, objectives, and policies. RN Case Managers work in nursing homes, home care, and in-patient settings (throughout various hospitals) and are assigned to a specific team: Home Care, Nursing Home, or In-Patient.

Responsibilities

  • Perform initial and ongoing patient/family assessments (physical, emotional, psycho-social, cultural, and spiritual) to coordinate comprehensive, holistic services by the Interdisciplinary Hospice Team.
  • Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
  • Document and enter all interventions (phone and home visits) into the EMR within 24 hours; medication changes by the end of the shift.
  • Develop and prioritize the interdisciplinary care plan goals with the patient/family, hospice team, and attending physician before team meetings.
  • Update the care plan at least every 14 days.
  • Provide anticipatory guidance to help patients/families prepare for and cope with events as they occur.
  • Teach, supervise, and counsel patients, families, and caregivers on physical care, including return demonstrations of procedures.
  • Communicate patient status effectively to family members and caregivers.
  • Revise nursing diagnoses, goals, interventions, expected outcomes, and plan of care based on assessment data and patient/family input before IDG meetings.
  • Report changes in patient/family condition and plan of care to the Coordinator, DPS, and On-Call staff.
  • Adhere to all state and federal laws and regulations outlined in policy and procedure manuals.

Requirements

  • Ability to work non-judgmentally with patients/families of any culture, religion, socio-economic background, or lifestyle.
  • Effective communication in English, both verbally (in person and by phone) and in writing.
  • Capability to work independently and cooperatively as a team member.
  • Skill in prioritizing and handling multiple tasks efficiently.
  • Strong organizational and time management skills.
  • Bilingual is a plus.
  • Valid New York State driver’s license with reliable transportation.

Work Environment

  • Work in various home environments, nursing homes, or hospitals contracted with Hudson Valley Hospice.
  • Moderate to high level of possible exposure to infectious agents.
  • Moderate amount of travel by car.
  • Moderate to high risk of exposure to unsafe environments.
  • Routine duties may involve direct exposure to blood or body fluids.

Qualifications

  • Graduate of an accredited nursing school.
  • Valid New York State nursing license.
  • Two years of nursing experience preferred.
  • One year of medical-surgical experience highly preferred.

Schedule

Monday–Friday (8:30 AM–4:30 PM); on-call rotation (1–3 times per year on average). FLSA: Exempt.

Pay

$80,000–90,000 per year.
