RN Case Manager

Holisticare Hospice is seeking a Full-Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout Delaware County, including Media, Wallingford, Broomall, and Newtown Square.

In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives care grounded in compassion, clinical excellence, and connection.

Schedule
- Monday - Friday, 8 am - 4:30 pm
Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner closely with our Clinical Director to assess the eligibility and suitability of clients for specialized hospice services.
- Provide direct care as an RN Case Manager, ensuring patient comfort and quality of life remain paramount.
- Deliver top-notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
- Empower patients and families through education, encouraging their active participation in creating personalized care plans.
- Participate in call duties to provide support when it matters most.
Requirements
- Current unencumbered license as a Registered Nurse.
- Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting.
- Hospice experience is a plus.
- Computer proficiency, including experience with EMR systems.
- Current CPR certification (if required by state).
- Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay
$96,000 - $100,000 per year (average pay range). Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position.
