About the role

Make a lasting difference as a Hospice Registered Nurse Case Manager (RNCM). In this rewarding position you will deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey as a valued member of an interdisciplinary care team.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests compassionately.
• Partner closely with the Clinical Director to assess eligibility and suitability of clients for hospice services.
• Provide direct nursing care that prioritizes patient comfort and quality of life.
• Deliver top‑notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
• Educate patients and families, empowering them to participate in creating personalized care plans aligned with their goals.
• Share call duties, ensuring support is available when it matters most.

Qualifications

• Current, unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical‑surgical/acute‑care setting (hospice experience a plus).
• Computer proficiency, including typing and EMR systems.
• Current CPR certification if required by state.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Pay

$94,000 – $99,000 per year (average pay range). Final compensation will be determined based on experience, qualifications, and specific responsibilities, and will be discussed during the interview process.
