RN Case Manager

Responsibilities

  • Administer skilled nursing care to patients requiring professional nursing service.
  • Teach and supervise family members and other nursing team members.
  • Provide and coordinate quality patient care, maintain a full caseload, and meet productivity goals with consideration of case mix.
  • Manage each patient’s care efficiently and cost-effectively to achieve positive outcomes regardless of payer.
  • Oversee the overall plan of care, including care delivered by LPNs and Home Health Aides (HHAs).
  • Conduct patient admission by:
    • Obtaining medical history related to the present condition from the patient and/or family.
    • Performing a physical examination, including vital signs, physical assessment, mental status, appetite, diet, ADL, and activity of daily living.
    • Evaluating the patient, family, and home situation to determine health teaching needs.
    • Assessing the patient’s environment to identify available family assistance.
    • Determining if home health aide services are required and their frequency.
    • Interpreting agency services to patients and families as part of care planning.
  • Meet regulatory requirements and understand various insurance guidelines.
  • Practice accepted nursing standards regarding infection control.
  • Develop and implement the nursing care plan, including:
    • Providing skilled nursing services, treatments, and preventive procedures as ordered by the physician.
    • Initiating preventive and rehabilitative nursing procedures for patient care and safety.
    • Observing and reporting signs, symptoms, and reactions to treatments, including drug effects and changes in physical or emotional condition.
    • Teaching, supervising, and counseling patients and caregivers on nursing care needs and related problems.
  • Oversee care provided by Home Health Aides by:
    • Supervising and evaluating care given by HHAs.
    • Documenting services rendered, changes in patient condition, and revisions to the care plan.
    • Recording supervisory visits, evaluating patient progress, and closing charts of discharged patients.
  • Utilize computer skills for accurate documentation.
  • Perform other duties as assigned.

Requirements

  • Associate’s degree in Nursing required; Bachelor’s degree in Nursing preferred.
  • Current, valid RN license issued by the NYS Department of Education.
  • Minimum one year of home care experience.
  • Knowledge of State and Federal regulations and payer guidelines.

Work Environment

The RN Case Manager primarily works in a non-office setting and may be exposed to outdoor conditions. The role involves medium work, including exerting up to 50 pounds of force occasionally and performing tasks such as lifting, carrying, pushing, or pulling objects, including the human body. The position requires sitting, standing, and movement.
