RN Case Manager
Job Summary
We are seeking an RN Case Manager to support patients and families through every step of their care journey. Your responsibilities will include guiding patients and families through program orientation, coordinating education, developing and implementing individualized treatment plans, completing psychosocial assessments, and collaborating with interdisciplinary teams.
Qualifications
- Associate Degree (APRN) or Registered Nurse (RN)
- Experience in case management or related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of rehabilitation philosophy and discharge planning
