Key Responsibilities
- Complete all necessary paperwork and initial assessments for new residents, including Level of Care determinations.
- Collaborate with multidisciplinary teams to coordinate safe and effective discharge plans.
- Conduct follow-up calls post-discharge and collect data for quality assurance.
- Monitor and organize OBRA forms monthly, ensuring compliance with recommendations.
- Attend meetings to discuss admissions and discharges, and participate in care conferences for residents.
- Aid the ADON in meeting the requirements for Post-Acute Care Certification.
- Schedule and review appointments with residents to ensure continuity of care.
- Perform additional tasks as assigned to support the rehab unit's operations.
Qualifications
- Education: Graduate from an accredited school of nursing.
- Licensure: Valid and current R.N. license in the state of Michigan.
- Experience: Minimum of two years of related experience, preferably in geriatric nursing.
- Skills: Strong interpersonal skills, knowledge of infection control and quality assurance activities, and the ability to establish effective working relationships.
- Physical Requirements: Ability to stand, walk, lift, bend, and sit as required in a busy healthcare environment.
