About the role
We are seeking a compassionate, grounded, and skilled Registered Nurse to join our Lynchburg team as a RN Case Manager. This role offers the opportunity to walk with individuals and families through the end-of-life and grief journey.
Responsibilities
- Provide case management and coordinate care for assigned patients and families
- Complete comprehensive assessments, admissions, and ongoing evaluations of patient needs
- Collaborate with the Interdisciplinary Team (IDT) to develop and implement individualized Plans of Care
- Deliver skilled nursing care including symptom management, IV therapy, wound care, and physical assessments
- Educate patients and caregivers on medications, pain management, and daily care needs
- Maintain accurate, timely documentation in compliance with hospice policies and regulations
- Provide emotional support and guidance to patients and families
- Participate in IDT meetings, on-call rotation, and quality improvement activities
Requirements
- Graduate of an accredited nursing program with an active Virginia RN license
- Minimum of 2 years nursing experience (hospice/home health or acute care preferred)
- Comfort with EMR and technology
- Reliable transportation with valid driver’s license and insurance

Qualifications
- Graduate of an accredited nursing program with an active Virginia RN license
