Captures patients through the acute care hospitalization process.

Responsibilities

• Manages all aspects of discharge planning/transition process
• Performs utilization management and quality screenings
• In-Patient Case Management is required

Requirements

• 2 years of acute care and In Patient Case Management Experience
• BSN required
• RN license required
• CCMC preferred
