RN Case Manager

About the role

Suncoast Hospice, a part of Empath Health, is seeking a caring and motivated RN Case Manager to work in Pinellas County on a Monday-Friday 8-5 schedule.

Responsibilities

• Manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Assesses the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, and environmental needs and activities of daily living.
• Collaborates with the Interdisciplinary Group (IDG) to develop, implement, and update the individualized plan of care.
• Initiates appropriate preventive and rehabilitative procedures and referrals to other services as needed.
• Educates patients, families, and caregivers about the disease process, palliative interventions, care giving, the dying process, and safety practices.

Requirements

• Graduate of an approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Bilingual preferred.
• CHPN certification within three years of employment, strongly encouraged.
• BSN or MSN preferred.

Qualifications

• One year experience in an acute-care Hospital environment providing direct patient care.
• Previous experience as a Hospice RN preferred.

Skills

• Strong communication and interpersonal skills.
• Ability to work collaboratively with interdisciplinary teams.
• Knowledge of hospice and palliative care principles.
