RN Case Manager
Responsibilities
- Provides end-of-life care and hospice services to patients in their home or community setting.
- Intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care.
- Assumes primary responsibility for the patient/family caseload, including assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team.
- Reports On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift.
- Maintains and completes appropriate care coordination notes for visit notes, and other pertinent clinical information.
- Affords assistance, provides, or arranges for delivery of medications and supplies needed after business hours, weekends, and/or holidays, as scheduled.
- Attends interdisciplinary team meetings during on-call period for full team discussion, consultation, and evaluation of patient/family needs.
- Collaborates with the triage team to admit patients eligible for hospice services.
- Develops patient care plan that specifically addresses identified patient problems, nursing problems, and goals that have been mutually agreed upon with patient(s) during on-call hours, weekends, and/or holidays.
- Ensures the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care.
Requirements
- Current unrestricted RN licensure in the state of practice.
- Valid driver’s license and proof of auto insurance.
- Two years of RN experience.
- Two years of Hospice/Palliative Nursing experience preferred.
