About the role
The RN Case Manager at Crossbridge Hospice provides end-of-life care and hospice services to patients in their home or community setting. This role involves providing intermittent skilled nursing services, directing, supervising, and instructing hospice aide staff, and maintaining care coordination notes.
Responsibilities
- Assume primary responsibility for the patient/family caseload during on-call hours, collaborating with the triage team.
- Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift.
- Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information.
- Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays, as scheduled.
- Attend interdisciplinary team meetings during on-call period for full team discussion, consultation, and evaluation of patient/family needs.
- Collaborate with the triage team to admit patients eligible for hospice services.
- Develop a patient care plan addressing identified patient problems, nursing problems, and goals mutually agreed upon with the patient(s).
- Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care.
Requirements
- Current unrestricted RN licensure in the state of practice.
- Valid driver’s license and proof of auto insurance.
- Two years of RN experience.
- Two years of Hospice/Palliative Nursing experience preferred.

- Excellent communication skills.
- Compassion and ability to support patients and families through sensitive and meaningful experiences.
Skills
- Ability to manage a caseload effectively during on-call hours.
- Knowledge of hospice and palliative care practices.
