RN Case Manager

Responsibilities
- Conduct comprehensive assessments of patients' medical, social, psychological, and financial needs to identify barriers to discharge and develop individualized care transition plans.
- Collaborate with physicians, nurses, social workers, insurance providers, and community resources to ensure continuity and quality of care.
- Advocate for patients and their families, ensuring their rights are respected, their preferences are considered, and they are empowered to make informed decisions about their healthcare journey.
- Manage patient caseloads, monitoring progress, evaluating the effectiveness of interventions, and making necessary adjustments to care plans to achieve optimal patient outcomes and efficient resource utilization.
- Educate patients and families on their health conditions, treatment options, available resources, and discharge instructions, preparing them for successful transitions to home, rehabilitation, or other care settings.
- Maintain accurate and timely documentation of all case management activities, assessments, interventions, and communications, ensuring compliance with hospital policies, regulatory requirements, and payor guidelines.
Requirements
- Nursing Degree from an approved school of nursing
- LICENSURE Active Registered Nurse license in good standing with the North Dakota Board of Nursing or Active Registered Nurse license in a compact state
- Working knowledge of patient care across various healthcare settings, care coordination principles, and utilization review
- Basic Life Support (BLS) Certification within the orientation period and meets re-certification as required
- Good communication skills necessary for constant communication, both oral and written, with patients, families, visitors, and medical center staff
- Accountable for own conduct and promotes and supports positive working relationships among staff, between other disciplines, departments, families, visitors, and patients
- Maintains confidentiality

- Experience in case management or related field preferred
- Knowledge of state nurse practice act
- Ability to work independently and as part of a team

Pay
The posted compensation range of $35.38 - $50.69 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Schedule
Monday through Friday 8-4:30, rotating weekends and holidays
