About the role
RN Case Managers coordinate and manage the care of high-risk patients, including those with multiple chronic conditions.
Responsibilities
- Meet with patients to manage their medical conditions
- Provide patient education
- Develop and implement a care plan
- Coordinate care with other clinical providers
- Collaborate with multi-specialty colleagues to ensure high-quality care
Qualifications
- RN License in the state of New Jersey
- BSN preferred
- 2 years of nursing experience, preferably in Case Management
- BLS certification
- Excellent interpersonal and computer skills
