About the Role

Brookhaven Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives care grounded in compassion, clinical excellence, and connection.

Schedule

Monday - Friday, 8 am - 4:30 pm

Territory

Bedford, Manchester, Milford, Merrimack and Amherst

Responsibilities

  • Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
  • Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.
  • Provide direct care as an RN Case Manager, ensuring patient comfort and quality of life remain paramount.
  • Deliver top-notch nursing care rooted in proven principles and techniques to ease pain and bring solace to patients.
  • Empower patients and families through education, encouraging their active participation in creating personalized care plans that align with their goals.
  • Participate in call duties to provide support when it matters most.

Qualifications

  • Current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Hospice experience is a plus.
  • Computer proficiency in typing and various programs, including experience with EMR.
  • Current CPR certification if required by state.
  • Current driver’s license, vehicle insurance, and access to dependable transportation.

Pay

$89,000 - $97,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position.
