Brookhaven Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. Schedule: M-F 8:00a-4:30p, etc. Territory: Wilmington, Andover, North Andover, Westford, Groton, Acton, Littleton, Ayer, Newburyport, Haverhill. In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection.

Responsibilities
