RN Case Manager

Start date: 8/24/2026 • Pay rate: $1,800.00 - $2,700.00

About the role

Responsible for initial assessment, utilization review, leading and documenting ITC meetings, patient/family interactions, setting up skilled home-care services, and coordinating necessary vendors for O₂, respiratory equipment, enterals, TPN, and hospital beds. Participate in care-plan meetings.

Requirements

• Active Massachusetts RN license
• AHA BLS certification
• Minimum 2 years of case-management experience

Schedule

8-week assignment • 8:00 a.m.–4:30 p.m. shift
