RN Case Manager

About the role

As a Home Health Aide, you will provide personal care services to patients in their homes, assisting with activities of daily living such as bathing, dressing, and eating.

Responsibilities

• Assist patients with hygiene and personal care tasks
• Monitor patients' health conditions and report changes to supervisors
• Help patients with medication management
• Ensure patient safety and comfort
• Document patient interactions and progress

Requirements

• High school diploma or equivalent
• Valid driver's license
• Pass a background check
• Ability to work flexible hours including evenings and weekends

Qualifications

• Experience in healthcare or related field preferred
• Strong communication and interpersonal skills
• Physical ability to perform manual labor and lift up to 50 pounds

Skills

• Excellent customer service skills
• Knowledge of basic first aid and CPR
• Proficiency in Microsoft Office applications
