About the role
We are seeking a Motivated RN to join our team in Utah County as a Case Manager.
Responsibilities
- Perform 60 day recertification assessments
- Conduct admission visits
- Supervise CNA activities
- Maintain routine nursing visits
- Coordinate care and provide care coordination
Requirements
- Active RN License
- Previous Home Health experience preferred
- Current BLS Certification
- Valid Driver's License and Vehicle Insurance
- Strong time management and prioritization skills
- Excellent communication, assessment skills and attention to detail
