RN-Case Manager

About the Role

The Registered Nurse (RN) is responsible for assessing, developing, and implementing individualized care plans to meet clients' health needs, ensuring quality and cost-effective outcomes.

Responsibilities

• Conduct timely OASIS data collection and monitor core measurements for compliance.
• Communicate with physicians and care teams regarding patient health status and treatment adjustments.
• Administer medications and provide patient and caregiver education.
• Coordinate interdisciplinary care, including supervising LPNs and Home Health Aides.
• Document all clinical activities accurately and participate in discharge planning.
• Maintain safety standards and ensure compliance with home health regulations.
• Supervise work assignments and manage performance of team members.
• Actively participate in quality and accreditation processes.

Requirements

• Current RN license in Florida.
• CPR certification.
• Valid driver’s license.
• Bi-annual completion of continuing education units (CEUs).
• Strong communication, clinical, and organizational skills.
• Familiarity with Prospective Payment System (PPS) and payor reimbursement guidelines.
• Thorough understanding of home health regulations and safety standards.

Work Environment

The position operates in office and home environments with potential exposure to biological hazards.
