RN Case Manager

Aegis Healthcare is expanding our Home Health and Hospice Nursing Teams in Scottsdale, AZ.

About the Role

As a Home Health & Hospice Nurse Case Manager, you'll be instrumental in providing exceptional care to patients in their homes. You'll collaborate with patients, families, and the Interdisciplinary Group to create and manage individualized care plans, ensuring a holistic care experience.

Responsibilities

• Conduct comprehensive physical, social, and psychological assessments.
• Collaborate with patients, families, and the Interdisciplinary Group to create individualized care plans.
• Manage care delivery and maintain meticulous patient records.
• Act as a resource for families, making proactive referrals.
• Perform patient transfers with a full range of movement.

Requirements

• Active Registered Nurse (RN) license in Arizona (or state of practice).
• Previous Home Health or Hospice experience (preferred).
• Effective verbal, written, and time management skills.
• Valid driver's license, reliable transportation, and proof of current auto insurance.
• Current CPR certification and negative TB test (assistance provided if needed).
• Valid AZ Fingerprint Clearance Card (assistance provided if needed).
• Ability to pass a criminal background check.

Pay

$40-$45 per hour.

Schedule

Full-time, Monday-Friday 8:00 AM-5:00 PM, with one 24-hour on-call shift per month.
