Responsibilities
- Develop and implement individualized care plans
- Monitor patient progress and adjust plans as necessary
- Coordinate with healthcare providers to ensure seamless care delivery
- Document all interactions and patient data accurately
Requirements
- Bachelor’s degree in Nursing or related field
- Current RN license in good standing
- Minimum 2 years of relevant clinical experience
- Strong communication and interpersonal skills
Qualifications
- Proficient in electronic health record systems
- Excellent organizational and time management skills
