Responsibilities

• Perform physical, psychosocial, and health system assessments to develop individualized plans of care
• Develop and implement a nursing plan of care and make recommendations based on patients' needs
• Request written orders from physicians for changes in the patient plan of care as needed

Requirements

• Graduate of an accredited School of Nursing (BSN preferred)
• Active New York State RN license; CPR-BLS preferred
• Minimum of two years of nursing experience (Medical-Surgical or Community Health; hospice or oncology preferred)

Pay

$100,000 - $116,000 per year

Schedule

Full-time, field-based role managing 3-4 patients per day.
