RN Care Navigator

About the role

RN Care Navigator, within the scope of the Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Registered Nurse Care Navigator, functioning in the spirit of teamwork, is accountable for the coordination and delivery of patient care utilizing the Nursing Process within the standards, policies, procedures and guidelines of the Organization. Specialty: Primary Care. Locations: Mount Carmel Medical Group Trivillage (2150 Marble Cliff Office Park Suite B, Columbus, OH 43215), Mount Carmel Medical Group Lewis Center (7100 Graphics Way, Ste 2400, Lewis Center, OH 43035), Mount Carmel Medical Group Worthington (81 E Wilson Bridge Rd, Worthington, OH 43085). Responsibilities

• Medicare annual wellness visits
• Outreach to patients
• Completing visits with patients
• Escalating concerns appropriately to providers
• Communication, education and scheduling of appointments
• EPIC

Requirements

Education: Graduate of a school of nursing; Associate's Degree required, Bachelor's preferred.

Licensure/Certification: Registered Nurse; current, unrestricted license to practice in the State of Ohio.

Experience: Minimum of two years experience in an ambulatory setting preferred; one year Case/Care Management experience preferred.
