RN Care Manager - Inpatient

Position: RN Care Manager Location: Glenview, IL Responsibilities Facilitate payor authorization and ensure accurate patient classification.Manage patient care collaboratively across the continuum, removing barriers to timely care and reimbursement.Ensure documentation reflects severity of illness and service intensity, working with the CDI team.Communicate effectively with physicians, nursing staff, and interdisciplinary team members to promote patient progress.Identify and resolve system issues that delay diagnosis, treatment, or discharge, optimizing length of stay.Coordinate with relevant disciplines to expedite care and facilitate patient discharge.Monitor patient progress, intervening as needed to ensure high-quality, efficient, and patient-centered care. Qualifications & Skills Current IL nursing license required.Bachelor's degree in Nursing; NLN-accredited preferred.Clinical certification (e.g., case management) is advantageous.Minimum of 2 years clinical nursing experience; experience in Utilization Management, Managed Care, or chronic disease management preferred.Knowledge of Electronic Medical Records and criteria systems like InterQual or MCG is a plus.
