RN Care Manager II-IPCM/FT/Mercy Hospital STL

About the role

The Care Manager is a member of the interdisciplinary team responsible for assessing, planning, advocating, and coordinating care from admission to discharge. This position ensures a safe transition post-hospitalization and maintains patient experience, safety, and quality of care.

Responsibilities

• Affirmatively assesses patients' needs and develops individualized care plans
• Advocates for patients' rights and preferences throughout their care journey
• Coordinates care across various departments to ensure seamless transitions
• Maintains accurate documentation and reports on patient progress and outcomes
• Collaborates with other healthcare professionals to optimize patient care
• Ensures compliance with regulatory requirements and best practices

Requirements

• Graduate of an accredited School of Nursing
• Bachelor's Degree of Science in Nursing, minimum required
• 2-3 years acute care hospital setting experience, preferred
• Care Management or Utilization Management experience, preferred
• Current License in the state of employment, required
• BLS (CPR) certification at hire date, required, or within 90 days of hire
• Certification in Case Management, preferred
