About the role
The Care Manager is a member of the interdisciplinary team responsible for assessing, planning, advocating, and coordinating care from admission to discharge. This position ensures a safe transition post-hospitalization and maintains patient experience, safety, and quality of care.
Responsibilities
- Affirmatively assesses patients' needs and develops individualized care plans
- Advocates for patients' rights and preferences throughout their care journey
- Coordinates care across various departments to ensure seamless transitions
- Maintains accurate documentation and reports on patient progress and outcomes
- Collaborates with other healthcare professionals to optimize patient care
- Ensures compliance with regulatory requirements and best practices
Requirements
- Graduate of an accredited School of Nursing
- Bachelor's Degree of Science in Nursing, minimum required
- 2-3 years acute care hospital setting experience, preferred
- Care Management or Utilization Management experience, preferred
- Current License in the state of employment, required
- BLS (CPR) certification at hire date, required, or within 90 days of hire
- Certification in Case Management, preferred
