RN Care Manager - Hospital

About the role

The RN Care Manager provides patient-centered care management services to patients and families within both the Emergency Center and inpatient settings. This position serves as an integral member of the interdisciplinary healthcare team and will be responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, patient and patient’s external support system (as applicable).

Responsibilities

  • Identify care coordination needs in collaboration with the inpatient and ambulatory care teams, patient and patient’s external support system (as applicable).
  • Progress patients towards discharge/next level of care by identifying barriers and implementing appropriate solutions/interventions.
  • Work with patients and patients’ external support system to secure appropriate and safe transition plans in keeping with shared goals of care.
  • Apply nursing values and continuous quality improvement in daily work.

Requirements

  • Current Minnesota Registered Nurse (RN) license.
  • Bachelor’s Degree in Nursing strongly preferred; would consider Associate’s Degree in Nursing with commensurate experience.
  • Minimum of one year hospital-based med-surg nursing experience preferred.
  • Prior experience in any of the following areas strongly preferred: utilization management, case management, home care setup, discharge planning.

  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficient in Microsoft Office Suite.
  • Knowledge of healthcare policies and procedures.

  • Effective written and verbal communication skills.
  • Ability to manage multiple tasks simultaneously.

Pay

Competitive salary based on experience and qualifications.

Schedule

Block schedule Monday-Friday. Rotating schedule to include every 6th weekend.
