RN Care Manager

As an RN Care Manager, you will be at the forefront of providing clinical care management services. What a Typical Day Will Look Like

• Conduct thorough assessments of clients’ conditions and monitor their well-being through regular visits (at least monthly).
• Collaborate with Geriatric Care Managers (GCM) to coordinate care and health-related services effectively.
• Complete Patient Review Instruments (PRI) and screens as needed.
• Manage medication regimens for clients, including pre-pouring medications and picking up prescriptions from pharmacies.
• Record vital signs and document each client visit meticulously.

• Utilize clinical expertise and compassionate communication to support clients and their families.
• Ensure accurate and comprehensive documentation of care and services provided.

Qualifications

• 5+ years of experience as a Care Manager or in a similar role preferred.
• Active RN license with a clean driving record and access to a vehicle (travel is required).
• Certification to complete Patient Review Instruments (PRI) and screenings is required.
• Strong clinical skills with experience in geriatrics and home health specialties.
