About the role
The Inpatient Registered Nurse (RN) Care Manager provides comprehensive, patient-centered care management for at-risk patients in an acute care setting.
Responsibilities
- Conduct assessments
- Develop care plans
- Coordinate care
- Manage resource utilization
- Review discharge plans
- Document interventions
- Ensure regulatory compliance
- Advocate for patients
Qualifications
- Associate's Degree in Nursing
- Valid Washington RN license
- At least 2 years of acute care experience in Medical-Surgical or inpatient settings
- One year of care management or utilization review experience (preferred)
- Knowledge of regulatory standards
- Excellent communication and organization skills
- Preferred candidates hold a Bachelor's Degree within three years and national certification in their specialty
