RN Care Manager

About the role

The RN Care Manager (Care Coordinator/ACO) works with Coulee Medical Center's medical teams to provide quality, comprehensive care to patients with complex health situations. Duties include individual screening, chronic condition(s) education, medical assessments, charting, care planning, stabilization, observation and maintenance, ongoing coordination of follow-up care, and support for an individual’s self-management.

Responsibilities

• Empower patients to self-manage their condition
• Reduce the occurrence of preventable complications
• Help patients stay out of the hospital
• Serve as the 'population health nurse' for CMC's ACO program

Requirements

• Current RN license to practice in Washington State
• Certified BLS

Qualifications

• Experience in managing patients with complex health conditions
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Skills

• Excellent organizational and time management skills
• Effective problem-solving and decision-making abilities
• Proficiency in electronic health record systems

Pay

The pay grade range for this position has been finalized for the facility according to an evaluation of the roles’ duties and requirements. The selected candidate will be placed within the appropriate range based on job knowledge, skills, education and experience.
