RN Care Manager
Start Date: 8/24/2026 • Pay Rate: $1,800.00 - $2,700.00
About the role
Provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. Enhance the quality of patient management and satisfaction, promote continuity of care, and ensure cost effectiveness through integrating case management, utilization review, and discharge planning. This role requires a highly organized professional with great attention to detail, adaptability to frequent change, and compliance with regulatory and departmental guidelines and policies.
Responsibilities
- Conduct comprehensive assessments of patient needs
- Develop, implement, and evaluate individualized care plans
- Coordinate case management, utilization review, and discharge planning
- Ensure continuity of care and cost-effectiveness
- Adhere to regulatory and departmental guidelines and policies
Requirements
- Nursing Diploma or ADN/ASN from an accredited school of Nursing
- NC/compact RN license
- 2 years of acute care hospital case management experience
- Care manager/discharge planning experience
Qualifications
- EPIC experience (preferred)
- Experience with educational/teaching hospitals (preferred)
- BSN or MSN from an accredited school of Nursing (preferred)
Schedule
13-week assignment • 8:00 AM – 5:00 PM
