RN Care Manager

About the role

RN Care Managers at EasyLiving Home Care and Care Management advocate for older and/or disabled adults and their loved ones to help make decisions related to healthcare, aging, and long-term care planning. They aim to:

• Reduce hospitalizations
• Improve quality of life
• Decrease the burden on the family or responsible party
• Recognize and advocate for the client's values and wishes, rather than their own

Responsibilities

RN Care Managers conduct, develop, monitor, and manage care plans and assessments to identify:

• Issues and problems
• Eligibility for assistance
• Need for services
• Onboard and sign up new clients, educating them on how our Care Managed Home Care (CMHC) services will help them age in place
• Review financial, legal, and/or medical issues and offer referrals and recommendations to specialists and other professionals
• Focusing on avoiding future problems and conserving assets
• Assist with determining the level of care needed and moving a client to a more appropriate facility or the arrangement of in-home care
• Act as liaison to responsible parties and professionals
• Facilitate our Care Managed Home Care model by:
• Managing intakes, completing on-site safety assessments, and creating care plans for new clients
• Providing coaching for caregivers with new clients to ensure success
• Oversight of the orientation and training to new and existing caregivers
• Complete documentation requirements accurately and timely
• Participate in marketing efforts with the goal of educating the public, cultivating clients, and securing new clients

Requirements

The ideal candidate will have:

• An RN license
• Experience in long-term care or home health care
• Strong communication and advocacy skills
• Ability to recognize and address client needs
• Knowledge of relevant laws and regulations
• Ability to manage multiple tasks and prioritize effectively

Qualifications

To qualify, candidates must:

• Have a current RN license
• Be proficient in Microsoft Office Suite
• Pass a background check
• Be able to travel to various locations within the Clearwater area

Pay

$30.00 - $35.00 per hour
