About the role
The Nurse Care Coordinator is responsible for overseeing care transitions throughout the healthcare continuum. They are an integral member of the multi-disciplinary team, focusing on ensuring high-quality patient outcomes, managing resource usage, and facilitating care delivery through collaboration and communication.
Responsibilities
- Develops and implements individualized discharge plans based on patient needs.
- Identifies additional services required and makes appropriate referrals.
- Facilitates multi-disciplinary rounds to ensure clinical readiness for discharge.
- Collaborates with the multidisciplinary team to manage throughput and mitigate delays in care.
- Works with physicians to determine appropriate lengths of stay and monitors resource utilization.
- Affirms patient needs and ensures timely implementation of discharge plans.
- Maintains accurate and comprehensive documentation.
Requirements
- Bachelor's Degree in Science in Nursing (BSN)
- One year of Registered Nurse (RN) experience in Missouri (NURSYS)
- BLS certification
- Missouri RN license
Qualifications
The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a collaborative environment.
Skills
Strong organizational skills, attention to detail, and the ability to prioritize tasks are essential. Proficiency in electronic health records (EHR) systems is also beneficial.

Schedule
Part-time, days (Saturday & Sunday), 8:00am - 6:00pm.
