RN Care Coordinator – Medical Oncology

About the role

Cleveland Clinic Hillcrest Hospital is a Magnet Hospital and part of the Cleveland Clinic network. The Department of Hematology & Medical Oncology provides specialized care for cancer and blood disorders. The role involves coordinating patient care and managing care transitions.

Responsibilities

• Collaborate with a multidisciplinary care team to develop and implement care plans.
• Identify patients with care coordination needs and conduct outreach.
• Conduct comprehensive clinical assessments and provide education and coaching to patients and families.
• Assist in managing transitions of care and connecting patients to community resources.
• Coach patients and families on self-management support and health maintenance tasks.
• Work with practices on quality and process improvement initiatives.

Requirements

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Bachelor’s of Science in Nursing (BSN) preferred.
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross.
• Three to five years of nursing experience.
• Strong attention to detail and therapeutic communication skills.

Qualifications

• Minimum qualifications: RN license, BSN preferred, three to five years of nursing experience.
• Preferred qualifications: BSN, Certified Breast Nurse or Certified Breast Navigator.

Skills

• Excellent communication and collaboration skills.
• Ability to manage complex patient cases.
• Proficiency in using healthcare technology.

Maximum Annual Salary: $96,467.50
