About the role
Cleveland Clinic Hillcrest Hospital is a Magnet Hospital and part of the Cleveland Clinic network. The Department of Hematology & Medical Oncology provides specialized care for cancer and blood disorders. The role involves coordinating patient care and managing care transitions.
Responsibilities
- Collaborate with a multidisciplinary care team to develop and implement care plans.
- Identify patients with care coordination needs and conduct outreach.
- Conduct comprehensive clinical assessments and provide education and coaching to patients and families.
- Assist in managing transitions of care and connecting patients to community resources.
- Coach patients and families on self-management support and health maintenance tasks.
- Work with practices on quality and process improvement initiatives.
Requirements
- Graduate from an accredited school of Professional Nursing.
- Current state licensure as a Registered Nurse (RN).
- Bachelor’s of Science in Nursing (BSN) preferred.
- Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross.
- Three to five years of nursing experience.
- Strong attention to detail and therapeutic communication skills.
Qualifications
- Minimum qualifications: RN license, BSN preferred, three to five years of nursing experience.
- Preferred qualifications: BSN, Certified Breast Nurse or Certified Breast Navigator.
Skills
- Excellent communication and collaboration skills.
- Ability to manage complex patient cases.
- Proficiency in using healthcare technology.
