About the role

A RN Care Coordinator-Complex Case Manager is needed in the Care Management department. The schedule is full-time from 10am-6pm. Three to five years of diversified clinical experience is required, with certification preferred.

Responsibilities

  • Understand confidentiality and legal/ethical issues related to patient health
  • Assess individual’s physical/mental impairment levels
  • Evaluate quality of necessary medical services and costs
  • Develop treatment plans and goals
  • Access and evaluate available resources to meet client needs
  • Partners with post-acute facilities to facilitate patient transitions
  • Propose solutions to senior leadership for patients with complex dispositions and potential insurance barriers
  • Document care management services
  • Exercise independent judgment in interactions with physicians, payers, and patients/families
  • Partner with post-acute facilities to move patients to appropriate care levels

Requirements

  • Minimum of 2 years of previous experience involving judgment and decision making, preferably in a utilization management/case management position
  • Minimum of 3-5 years recent clinical experience required, and 1-3 years of recent acute, Inpatient Care Management experience required
  • Knowledgeable on accessing and evaluating available resources to meet client needs
  • Strong organizational and time management skills, as evidenced by the ability to prioritize multiple tasks and role components

Skills

  • Excellent interpersonal, verbal, and written communication and negotiation skills
  • Strong analytical, data management, and PC skills
  • Current working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement
  • Understanding of pre-acute and post-acute venues of care and post-acute community resources
  • Ability to work independently and exercise sound judgment in interactions with physicians, payers, and patients and their families

Pay

$85,176.00 - $196,747.20

Schedule

Full Time, 10am-6pm
