About the role
The Care Coordinator at Saint Luke’s East is responsible for overseeing care transitions throughout the healthcare continuum. They collaborate with a multidisciplinary team to ensure high-quality patient outcomes, manage resource utilization, and address any obstacles to care delivery.
Responsibilities
- Develops and implements individualized discharge plans based on patient needs.
- Identifies additional services needed and makes appropriate referrals to external resources.
- Facilitates multi-disciplinary rounds to discuss patient care and discharge plans.
- Maintains clinical readiness for discharge and collaborates with the multidisciplinary team to ensure timely implementation of discharge plans.
- Works with patients, families, and caregivers to ensure their needs are met and appropriate discharge plans are implemented.
- Maintains accurate and thorough documentation of patient care activities and outcomes.
Requirements
- Bachelor's Degree in Nursing (BSN) required within 3 years of starting the position.
- One year of applicable experience.
- Basic Life Support certification from American Heart Association or Red Cross.
- Registered Nurse (RN) license.
