About the Role

As our Staff Nurse, you will be a pivotal healthcare professional, delivering compassionate, high-quality care that impacts our patients' well-being and recovery. You will leverage your expertise to provide individualized, comprehensive care, making critical assessments, performing skilled procedures, and implementing patient care plans. You'll collaborate within an interdisciplinary team focused on optimal patient outcomes.

The RN Care Coordinator is responsible for assessing, planning, facilitating, and advocating for options and services through a continuum of care from point of contact through discharge. This role emphasizes care coordination, communication, and collaboration with nursing staff, physicians, departments, insurers, and post-acute service providers to achieve optimal outcomes within the appropriate level of care.

The RN Care Coordinator will advocate for patients and families by identifying and addressing patient choice, spiritual needs, cultural, language, and socioeconomic barriers to care transitions. You will protect confidentiality while striving for high levels of patient satisfaction and conduct utilization review and discharge planning processes in compliance with hospital policies, standards of practice, and federal and state regulations.

Responsibilities

  • Deliver compassionate, high-quality patient care through individualized assessments and skilled procedures.
  • Implement and manage patient care plans in collaboration with an interdisciplinary team.
  • Assess, plan, facilitate, and advocate for patient care options and services from admission through discharge.
  • Coordinate care with nursing staff, physicians, departments, insurers, and post-acute service providers.
  • Identify and address patient and family needs, including cultural, spiritual, language, and socioeconomic barriers.
  • Protect patient confidentiality and ensure high levels of patient satisfaction.
  • Conduct utilization review and discharge planning processes in compliance with hospital policies and regulations.
  • Comply with Dignity Health Standards of Conduct, policies, and procedures, and assist in risk evaluation and mitigation.

Requirements

  • Minimum of 3 years of nursing experience in an acute hospital setting.
  • Prior Care Coordination experience in a clinical or insurance setting.
  • Registered Nurse (RN) license.

A Master’s Degree in nursing with a concentration in Case Management can substitute for the experience requirement. BSN degree or equivalent experience preferred. CM certification is also preferred.

Pay

$80.77 - $104.48 per hour.
